O Hospital de Custódia e Tratamento Psiquiátrico (HCTP) é, em sua essência, ambivalente e ambíguo, já que agrupa não só características hospitalares como também características de presídio, simbolizadas pelo sistema de segurança. Ao analisar tal contexto torna-se perceptível a importância da implementação de um saber-fazer capaz de atender às reais necessidades das pessoas encerradas nessa instituição. Essa leitura de necessidades deve supostamente estar vinculada a um trabalho em saúde mental pautado nos princípios da Reforma Psiquiátrica e na Prática do Campo Psicossocial. O objetivo da presente discussão é refletir acerca das reais possibilidades da implementação do trabalho em saúde mental pautado nos princípios da Reforma Psiquiátrica Brasileira, inserido no Campo Psicossocial, em instituição como HCTP. Essa reflexão se dará a partir dos conflitos surgidos na implantação do serviço de Terapia Ocupacional em um HCTP no estado de São Paulo e também através da análise das dimensões da realidade à qual o HCTP pertence. Analisando a Lei da Reforma Psiquiátrica (lei n. 10.216/2001), o decreto n. 28.195/1988, que delibera as funções da Terapia Ocupacional nos HCTP do estado de São Paulo, e a Lei de Execução Penal, fez-se a análise da realidade a partir de suas dimensões, para entender que forças institucionais regiam os processos de trabalho das terapeutas ocupacionais. Assim, foram capturadas as dimensões estruturais, particulares e singulares que regem o HCTP e após tal captura foram revelados os “nós” que impedem a implementação de um trabalho em saúde mental no campo psicossocial dentro de tal instituição.
