A partir das transformações no campo da Saúde Mental, advindas do movimento da Reforma Psiquiátrica Brasileira, foram formuladas novas diretrizes para ações, de maneira a procurar responder às diferentes demandas dos sujeitos atendidos, aliando inclusão social ao acompanhamento clínico. Nesse contexto, o Acompanhamento Terapêutico (AT) configura-se como dispositivo ímpar para a composição de intervenções voltadas às demandas singulares de pessoas com transtornos mentais. Diante disso, o presente trabalho visou a construção de reflexões sobre as limitações e potencialidades do AT. O estudo foi conduzido a partir da prática do AT junto ao estágio de Terapia Ocupacional em Saúde Mental da Universidade Federal do Paraná. Enquanto método, o artigo estrutura-se em dupla via: apresentação de relato de caso e pesquisa exploratória realizada a partir de entrevistas não estruturadas focalizadas com dois sujeitos acompanhados em AT. Para o tratamento dos dados utilizamos a análise hermenêutico-dialética. Os resultados apontam ganhos significativos para o processo terapêutico a partir da utilização do AT, possibilitando ações focadas em demandas do indivíduo, família e comunidade. Contudo, há a necessidade de continuidade das ações empreendidas pelo AT, assim como de superação de impasses estruturais, que tendem a diminuir a potência do dispositivo.
